    Remember, all plan changes must be turned into Brian Nesbit, Payroll, by June 30.

    -If you are switching from WPS to GHC, or GHC to WPS, contact kgebhardt[at] matcmadison [dot] edu (Kristin Gebhardt) or bnesbit[at] matcmadison [dot] edu (Brian Nesbit) for enrollment forms.

    -If you are switching from WPS Statewide to WPS Southern, complete the Southern Election form.

    -If you wish to stay with your current health insurance choice, no action is needed.
