Enhanced Membership Service Hours Form

Benefits:

Each semester you will get:

• a certificate, signed by the Chapter President and Advisor;
• an invitation to the Enhanced Member Banquet;
• a gift based on a number of semester:

                                       First:              Plaque and name plaquer (if you did not go to Inductions);

                                                             just the name plaquer ( if you went to Inductions)

                                       Second:          Stole and tassel

                                       Third:             Something from Phi Theta Kappa Store up to $20.00

                                       Fourth:           Something from the Phi Theta Kappa Store up to $25.00

Eligibility

To be an Enhanced Member you need to do the following each semester:

• have at least 30 hours of community service either with PTK or within the community;
• volunteer on two PTK events;
• attend at least one General Meeting.

Service Hours Recording Periods

The year is broken into 2 recording periods:

• May 1st -- December 18th;
• December 19th -- April 30th.

The form for recording hours is due on May 4th for the first half of the year and December 19th -- for the second.

Types of Service

Service hours fall into 2 categories: PTK-related and community service.

PTK related service includes participating in PTK-sponsored activities, such as:

• attending meetings and programs;
• helping with sponsored events;
• joining some of our fellowship events;
• any service related to the international service topic " The Democratization of Information."

 If you are not sure if something qualifies, ask one of the officers or Chapter Advisor.

Turning in Forms

You may submit Service Hours Form (PDF, 40 KB) anytime before the end of the recording period, once you have reached the minimum required 30 hours. Submit your completed forms to Renee Alfano, PTK Advisor, or Carl Peterson, VP of Service:

• bring them to Student Life office at Truax campus;
• mail them to Renee Alfano at Madison Area Technical College, 3550 Anderson St., Madison, WI, 53704;
• email them to ralfano[at] matcmadison [dot] edu
