Petition Ready Requirements
To be eligible for entry into the nursing core courses and to submit a petition, the following requirements must be completed:
- Priority Date assigned after application, review and $50 deposit paid.
- Completion of the following General Education Courses with a "C" or better in all courses:
- 10-801-195 Written Communication OR 20-801-201 English Comp 1
- 10-801-198 Speech OR 10-801-196 Oral/Interpersonal Communication OR 20-810-201 Fundamentals of Speech
- 20-809-203 Introduction to Sociology OR 10-809-197 Contemporary American Society
- 20-806-107 Anatomy & Physiology 1
- 20-806-208 Anatomy & Physiology 2
- 20-806-273 Microbiology
- 20-809-231 Introduction to Psychology
- 20-809-233 Developmental Psychology
- 99-999-999 Elective Courses total 5 credits
- Completion of TEAS preadmission exam, benchmark scores achieved
- Completion of Nursing Assistant training
- Completion of the Petition Process Tutorial
- Documentation of current Health Care Provider CPR certification
- Completion of the Functional Abilities Form (available March 2010). Review current list of Functional Abilities for nursing.
- Completion of the Criminal Background Check (CBC) Please Note: Completing the CBC does not guarantee clinical placement. Disclosure Form & Release forms are required.
- Completion of the Policy Signature document.
Petition Instructions
- Only those students who are prepared to begin core nursing courses should submit a petition packet during the designated petition window. Refer to the the Petition Process tab on the Associate Degree Nursing Program Page.
- Complete the Petition Process Tutorial (available March 2010)
- Download the Petition Form (available April 2010) from the Associate Degree Nursing web page.
- Students must petition to a specific semester, campus and class section. Students may petition to only one campus per semester
- Submit all the required documents (regardless of how many times you have petitioned)
- Petition packets must be turned in or mailed in a sealed envelope to the Center for Health and Safety Education Office (CHASE) no sooner or later than the designated petition window. NO EXCEPTIONS.
- Mailed Petition packets must be received (not postmarked) by the petition deadline date.
- It is recommended that students make a copy of their petition packet.
- Your petition will not be considered unless ALL REQUIREMENTS ARE MET AND ALL DOCUMENTS ARE SUBMITTED AS REQUESTED.
- If a student’s petition is accepted and they choose not to begin, their petition will be voided. The student will have to reapply to the program and receive a new priority date.
- All general education courses must be completed with a grade of "C" or better before you may submit a petition packet.
- Deferments to another year or semester is not an option.
Notification
- Students will be notified of their status via letter within 45 days of close of the petition window.
- Accepted students will be notified of the date and time to attend a mandatory on campus program orientation/workshop.
- Accepted students that do not attend the mandatory orientation, do not register by their registration deadline or drop the classes will forfeit their seat.
- Non-accepted students are encouraged to petition again during a future petition window.
- All decisions regarding acceptance or non-acceptance are final and cannot be appealed. Madison College reserves the right to hold all decisions final.
