Agreement for The Mitby Theater
The policies stated in the "Mitby Theater Rental Policies and Procedures
Manual" are considered to be conditions for the rental of MATC's Mitby
Theater at 3550 Anderson Street, Madison, Wisconsin. The Renting Organization
also acknowledges that this agreement is subject to all the policies,
rules and regulations of the MATC District and agrees to conform to
those policies, rules and regulations, and to the Laws of the City of
Madison and of the State of Wisconsin. Any violation of law or policy
will result in either the revocation of the contract or additional payments
as determined by the MATC District Board of Directors or their designee.
Acting as the authorized agent for the renting organization, I have
read, understood and agree to follow the guidelines, regulations and
policies outlined in the "Mitby Theater Rental Policies and Procedures
Manual and the additional requirements contained in the facility use
agreement to follow. By my signature, I acknowledge and accept the obligation
to pay the charges incurred by my organization if granted the use of
