Address Change
Notify the Enrollment Center, (608) 246-6210, or the administration office of each regional campus with any address and phone number changes. The college must have up-to-date information on file in the event of an emergency and for the mailing of grade reports and other correspondence. The student’s address should reflect the location at which they wish to receive all correspondence.
Name Change
If a student’s name changes, first or last, and they want this change reflected on their academic record, they need to complete and sign a Change of Name Declaration Card or the “demographic” portion of the student study list. Students can get these forms from the Enrollment Center. Students changing both their first and last name will be required to provide at least one legal form of documentation.
