Procedures for Faculty
The qualified student with a disability must present you two pieces
of information to receive note-taking services.
- Accommodation
Card from Disability Resource Services. (Review and give back to
the student.)
- Note-taking request form (Check appropriate option
regarding notes.)
Note: A student must provide you with both documents to utilize
note-taking services in your class. If a student does not, please refer
them to Disability Resource Services, Truax 159 or DTEC 109.
- Once
the student presents you with the aforementioned documents, please
discuss how to best make the note-taking accommodation with the student.
Faculty has two options regarding note-taking:
- Provide
a student with a copy of your own lecture notes. If you select
this option, please return this form promptly to DRS.
- Solicit
a student note-taker from the class.
- If you are not going
to provide your notes, make an in-class announcement regarding
the need for a student note-taker to the class. A student is reimbursed
$50 a credit for his/her notes. Note: If
the student takes notes for more than 1 student in the class, he/she
will be reimbursed $25 a credit for these additional students. Please
keep the student's name confidential when making the announcement.
- If
no one volunteers in the first class period, please make the in-class
announcement at the beginning of the second class.
- Give
the student note-taker the note-taking request form and send them
to Disability Resource Services (Truax 159; DTEC 109).
- Notify
Disability Services (246-6716) of any difficulties in locating
a note-taking volunteer or with any other concerns.
