5. Credit from previous
college or university courses - One of the eligibility criterion for
a TOP Grant is that you must be a "first-year student," which
means that you have not earned, received, or completed more than 18 postsecondary
credits (whether or not those credits apply to your intended program) after graduating from high school or earning an HSED. Courses
completed before graduating from high school or earning an HSED do not
count towards the 18-credit limit. Mark the appropriate box if you
have taken any college courses and indicate the number of credits earned.
6. Certification of disability
- The TOP Grant program considers a student with a disability (that significantly
restricts the student's ability to learn compared to the average person
who has comparable knowledge, skills, and abilities) to be enrolled full-time
if the student enrolls in and completes at least 6 program credits per
semester. A qualified third-party professional such as a doctor, physician,
rehabilitation vocational counselor, high school district, or counselor
at the Division of Vocational Rehabilitation must determine the student's
disability. The district must verify this disability determination. In
section 6a, the district special services coordinator or designee must
certify that his or her office has hard-copy documentation of the student's
disability and that that disability significantly restricts the student's
ability to learn compared to the average person who has comparable knowledge,
skills and abilities.
7. Previous TOP Grants received
- Indicate whether you have ever received a TOP Grant and if so, how many.
A maximum of four grants of $250 each may be received, up to $500 per
school year.
8. Certifying statement
and signature - Read the certifying statement before signing the TOP
Grant Application form. Indicate the term, spring, fall or summer, and
note the year you plan to attend. You must sign and date the TOP Grant
Application form. Failure to do so may void your TOP grant eligibility.

Continued Eligibility:
If a student fails to meet the grant eligibility requirements during a
semester for which he or she received a grant, the student will be placed
on grant probation for the following semester. While on probation, the
student is ineligible for a TOP grant. If the student meets the eligibility
requirements during a probationary semester, the student may apply for
any remaining grant eligibility. If the student fails to meet the grant
eligibility during a probationary semester, he or she forfeits any remaining
TOP grant eligibility.
Probation Exemptions:
Under special circumstances, students may apply for up to two probation
exemptions, during which they do not lose their remaining grant eligibility.
Mid-Semester:
A student who does not expect to be able to satisfy the TOP Grant requirements
for a semester grant they have already received because of some personal
hardship reason may apply for a TOP Grant probation exemption before the
end of the semester. A probation exemption granted before the end of the
semester exempts a student, who has received a grant, from going on grant
probation in the subsequent semester.
Pre-Semester:
A student who does not expect to be able to satisfy the TOP Grant requirements
for a semester following a semester in which they received a TOP Grant
because of some personal hardship reason, may request an exemption from
the TOP Grant consecutive requirement.
