Reimbursement: $100 per member per year (or $200 per family of two or more enrolled members)

Includes reimbursement for:

• Health club membership (minimum 120 workouts)
• Community Supported Agriculture (CSA)
• Weight Watchers and Jenny Craig
• Community education classes at UW Health, St. Mary’s, and Meriter
• Doula services (pre- and post-natal support)
• Athletic shoe reimbursement

WPS Healthsense Wellness Reimbursement

Deadline: within 16 months of activity/service
Reimbursement: up to $200 per member per year (spouse enrolled in family plan can also receive $200)*

Includes percentage reimbursement:

• Health club memberships
• Community education classes
• Smoking cessation programs
• Community Supported Agriculture (CSA)
• Weight Watchers
• Select wellness programs
