
Hiring Document Instructions
Instructions for completing the hiring documents are below. PDF or DOC versions of these forms may be downloaded and printed. Bring or send a hard copy of all completed forms and the required documentation to the tutoring office, room 232A, Truax campus.
1. Tutor Information Form (DOC, 31KB)
Complete this form, then sign and date it. List all courses you wish to tutor.
2. Employment Application (DOC, 61KB)
Complete the Employment Application; sign and date.
If you are a Madison College student carrying 6 credits or more you must also complete the FAFSA (Free Application for Federal Student Aid) available from the Financial Aid Office.
3. I-9 Employment Eligibility Verification (PDF, 408KB)
Complete Section 1, sign and date.
Provide a copy of your driver's license and social security card. (See second page for alternatives.)
4. Federal Form W-4 (2012) (PDF, 99KB)
Complete the bottom section of this form - items 1 through 7, sign and date.
5. Payroll Electronic Deposit Form (PDF, 180KB)
Complete this form, sign and date. All paychecks are electronically deposited. NOTE: You must provide a voided check or deposit slip for the account you wish to use and attach it to the payroll Electronic Deposit form. If you do not have an account, contact our office for information on other options.
6. Madison College Statistical Reporting Card (DOC, 22KB)
Complete this form, sign and date. Please note this form is for statistical reporting purposes only.
7. Reference Letter (DOCX, 204KB)
Fill in your name and student ID number and have your reference complete the remaining information.
8. Peer Tutor Guidelines Form (DOCX, 60KB)
This document covers many of the basic rules and expectations for Peer Tutors. Please read and sign this document. You may wish to print a copy for your information.
NOTE: You cannot begin tutoring until we have received and processed all documents.
