MATC Facilities Management Work Request Form

For requests needing IMMEDIATE attention please call:   
x6020 for Maintenance or x6030 for Custodial

Instructions:  This form is to be used to request non-emergency custodial and/or maintenance work to be scheduled by the Facilities Department.  After regular hours (7:30 am - 5:00 p.m. Monday through Friday) or anytime the office (x6020) is not staffed, our answering service will answer you call. You will then be asked if your call is an emergency that requires an on-call person to be paged immediately or if you can leave a message that will be forwarded to the staff.  Messages are retrieved three times during the day, starting at 6:00 am.

Type of Request: Select the category that best describes your request.  If nothing is applicable select "other".
Contact: Identify the contact person
Phone: Identify the contact person's phone number (at least 4 digits)
Work Requested:

Desired Completion Date: Indicate the date you would like the work to be completed.
Chart Field number: If the cost of the completed work will be charged back to your department, please provide the Chart Field number.

  

When you click on the "submit" option, a form confirmation page will show you exactly what is sent to the MATC Facilities Department for processing.   Based on your request and needs, a work order will be issued to appropriate Facilities Management staff to follow-up on the request.   

Note:  As soon as the request is processed, a hard copy of the issued work order is sent to you in inter-office mail.  The copy of the work order will have the anticipated completion date marked on it.  Due to the number of work orders we process, we do not send out copies of the finished work order. We would ask that the requestor check the work requested on, or shortly after, the date the work was to be done and notify Facilities job planner if the work was not done.  If the work is of the type where the requestor can not check if work was done the requestor can note on the original work request that they would like to have confirmation of the work being completed.
