Procedure, Orientation Form and Checklist

Purpose: To ensure that all employees of MATC receive proper training and information regarding accident reporting, emergency evacuations and emergency procedures, and any hazards pertaining to their specific workplace/assignments.

Overview:This safety orientation and checklist have been developed to help employees and supervisors understand the health and safety training requirements. The "Employee Health and Safety Orientation" form should be reviewed and completed for each new emplyee.  The Health and Safety Training Checklist is provided to assist in identifying special health and safety training needs.   

Responsibilities

Supervisors/Managers

  • Assures that no employee begins any work assignment without first reviewing applicable EH&S procedures and policies, as per the Health and Safety Training Checklist with all new employees prior to them starting work.
  • Completes the "Employee Health and Safety Orientation" form with each new employee.  This form is a signed acknowledgement from each employee certifying that he or she has reviewed and understands the applicable health and safety requirements.
  • Maintains the original "Employee Health and Safety Orientation" form and sends a copy to the Safety Office.

Note: supervisors/managers may complete the "Employee Health and Safety Orientation" form (and checklist) with the new employee, or may send the employee to the Safety Office to have this done.   In either case, it is the supervisor/manager's responsibility to ensure that the employee receives the appropriate safety orientation and that the orientation activities are documented.

Safety Manager

  • Assists in providing /coordinating necessary health and safety training for employees.
  • Maintains documentation of Employee Health and Safety Orientations.

To complete the Employee Health and Safety Orientation

  1. review the Health and Safety Training Checklist (PDF, 102 KB) with employee. complete the Employee Health and Safety Orientation (PDF, 97 KB) form with employee. arrange for additional health and safety training needs, as appropriate.
  2. send a copy of the completed documentation to the Safety Office.
