Available for download in both Word and PDF formats, Phi Theta Kappa forms may be accessed by utilizing the provided links in the gray box on the right of the page. Forms not requiring a signature may be emailed back to the Phi Theta Kappa office at : http://matcmadison.edu/ptk. All others may be obtained and returned in person at our office in room 131 or the Student Life office on the Truax campus. Since our office is staffed by fellow student volunteer members, handling unwarranted forms only slows our efforts. Provided for your convenience, we ask that on line forms be used only as necessary. Below are descriptions of each of the forms pertaining to membership in Beta Beta Psi.
New Member Forms:
Beta Beta Psi Personal Inventory Form
This inventory form is for our local chapter. It reflects member's interests, skills, and talents. It also allows us to know how to contact you.
Ceremony Class Excuse Form
Complete this form if you need to leave class early or miss class because of an induction ceremony and would like a memo sent to your instructor.
Mentor Recognition Form
Complete this form if you would like to nominate an instructor for recognition at the induction ceremony.
Membership Forms:
Member Profile Information Update Form
If you are a current member, please use this form to update your contact information to that you may be contact about new information for the future.
Graduation Stoles and Tassels Form
This form is used to order the graduation stole and tassels, for members who are graduating at the end of the current semester. This form will be mailed out to you.
Enhanced Membership Forms:
Service Hours Form
This form is used to track service hours earned toward enhanced membership. Requirements and time periods are shown on the form. When printing this form you do not need to print the instructions page; the form itself is all that needs to be turned in. Don't forget to fill out the Activity Sheet for all your Phi Theta Kappa related hours.
Activity Sheet Form
This form is to be filled out by each Phi Theta Kappa member that attends an event and wishes to have the event count toward Enhanced Member Hours. These forms need to be returned to our office; we maintain a file of these forms throughout the academic year so that we may refer to them in order to complete our Hallmark essays and compile our Year Book.
